Special Programs
AHCCCS ENROLLMENT
Our
financial counselors can help people with limited income to enroll
in AHCCCS over the internet. This provides faster processing and
shorter wait times. There is no separate interview with an AHCCCS or
DES caseworker needed.
FAMILY HEALTH PLAN/SLIDING FEE PROGRAMS
Family
Health Plan
is designed
to make quality health care available to people who lack adequate
insurance and have limited income. Qualifying members of the
Family Health Plan
have access
to the quality health care services at Canyonlands Urgent care and
Lake Powell Medical Center at a reduced rate. Based on individual
financial circumstances, the
Plan
allows
members to pay variable rates from a minimum of $15.00, up to 75% of
the office visit charges.
There is no
charge for membership in the
Family Health Plan.
However, it
is available only to those who meet the eligibility requirements. We
recommend all patients who meet the requirements apply for the
Family Health Plan,
regardless of current insurance coverage, as the
Plan
may serve as a secondary payer on the balance due after insurance.

to see our Patient Services Representative. To qualify for the
Family Health Plan,
an
application must be completed and Proof of Income (most recent tax
return, check stubs, Affirmation of Income, etc.) must be provided
for verification.

Quality Health Initiatives
WWHC
The Well Woman’s Check program is for women
over 45 years of age who are uninsured or under insured.
Qualifications are based on family income. The WWHC consists of a
yearly visit, pap smears, clinical breast exams, and a mammogram.
Additional tests may be covered if necessary.
RADIATION SCREENING
RESEP Program, Radiation Exposure Screening
and Education Program provides health screening at no out-of-pocket
cost for qualifying patients who were exposed to above ground
radiation testing and uranium mining.
PRESCRIPTION ASSISTANCE
This program provides assistance with
prescription coverage for patients who have no prescription
coverage. There are eligibility requirements. Please bring your
medications to every visit.
IMMUNIZATIONS
We offer
infant, child, adolescent, and adult recommended immunizations.
Immunizations help protect against diseases and are one of the most
important ways that people can protect themselves and their loved
ones against serious diseases.
Children birth
through 18 years of age may be eligible for free childhood
immunizations through the Vaccines For Children Program. This
program offers recommended immunizations for children, who are on
AHCCS or KIDSCARE, have no other insurance, are Native American, or
have no immunization benefits on their health insurance plan. Those
with private insurance that fully covers immunization services are
not eligible for the Vaccines For Children Program.
Please bring
your Immunization Record Booklet to every clinic visit to ensure the
record is complete and up-to-date.
HEALTH
EDUCATION
CHRONIC CARE FOR DIABETES, ASTHMA AND HYPERTENSION
Information is available about nutrition
guidelines, exercise, self care at home, monitoring medications, and
disease prevention. The goal is to improve patient control of their
health condition.
CHILDBIRTH EDUCATION
Prenatal classes are free, they last 5-6
weeks and are held on Tuesdays from 5:30-7:30 at Lake Powell Medical
Center. The class consists of five sessions. Expectant parent and
birthing partner are encouraged to sign up for the class that is at
least six weeks before their due date. Please call 928-645-8123 to
sign up.
BREASTFEEDING
Classes are free and are taught by a
Certified Breastfeeding Counselor. The classes are once a month on
the second Thursday of every month from 5:30-7:30pm at Lake Powell
Medical Center. Please sign up in advance by calling 928-645-8123

The Mission of Canyonlands Community Health Care is to
provide community health centers that deliver quality,
comprehensive, culturally sensitive, primary and preventative health
care, which is available for all those who reside or visit the areas
we serve. We currently have two community health centers in
Page—Lake Powell Medical Center and Canyonlands Urgent Care. We are
also located in Fredonia, Littlefield, Kaibeto and Chilchinbeto,
Arizona as well as Overton, Nevada.
